Understanding Bioidentical Hormone Therapy: A Provider Overview

"Bioidentical" refers to hormones that are chemically identical to those produced by the human body. Understanding what the term does and does not imply helps providers and patients make informed decisions about hormone replacement therapy.

Defining Bioidentical Hormones

Estradiol, progesterone, and testosterone can all be produced in bioidentical forms. Some FDA-approved products contain bioidentical hormones, and others are compounded by pharmacies according to a provider's prescription. The term describes molecular structure; it does not by itself indicate that a product is safer or more effective than another.

Clinical Considerations

  • Indication: Therapy should be tied to a clear clinical indication, such as menopausal symptoms or confirmed testosterone deficiency.
  • Baseline evaluation: History, examination, and laboratory testing appropriate to the patient and the hormone being considered.
  • Individualization: Route, dose, and formulation should reflect patient preference, risk factors, and response.
  • Informed consent: Patients should understand expected benefits, known risks, and the limits of current evidence.
  • Pharmacy standards: Prescriptions should be filled by licensed pharmacies that follow applicable federal and state requirements.

Delivery Methods

Hormone therapy can be delivered orally, transdermally, by injection, vaginally, or by subcutaneous pellet. Each has different pharmacokinetics, convenience, and risk considerations. Providers choose the method that best fits the individual patient and their own clinical experience.

Monitoring

  • Symptom response and adverse effects at defined follow-up intervals
  • Laboratory monitoring appropriate to the hormone used
  • Age-appropriate screening and cardiovascular risk management
  • Periodic reassessment of whether therapy remains appropriate
